Description:

The Vorrithan is an ancient fiend birthed from the Abyss itself, a floating mass of pulsing flesh and twisted bone. Its many mouths drip with ichor, and its deep, resonating voice carries whispers of corruption. A creature of malice and grotesque artistry, the Vorrithan takes pleasure in warping the flesh and minds of its enemies, reveling in their despair as their very forms betray them.

Traits

Aura of Corruption. At the start of each creature’s turn within 30 feet of the Vorrithan, it must succeed on a DC 18 Constitution saving throw or take 10 (3d6) necrotic damage and gain 1 level of exhaustion. Creatures immune to exhaustion are unaffected.

Fleshwarping Mastery. The Vorrithan can use its action to corrupt the flesh of a creature within 60 feet that it can see. The target must succeed on a DC 18 Constitution saving throw or suffer a random effect from the Fleshwarping Table (see below).

Magic Resistance. The Vorrithan has advantage on saving throws against spells and other magical effects.

Legendary Resistance (3/Day). If the Vorrithan fails a saving throw, it can choose to succeed instead.

Actions

Multiattack. The Vorrithan makes two Corrupting Ray attacks.

Corrupting Ray. Ranged Spell Attack: +9 to hit, range 120 ft., one target.
Hit: 27 (6d8) necrotic damage. If the target is a creature, it must succeed on a DC 18 Charisma saving throw or suffer disadvantage on all saving throws until the end of its next turn.

Unholy Ichor (Recharge 5–6). The Vorrithan spews forth a pool of corrosive ichor in a 20-foot radius centered on itself. Each creature in the area must make a DC 18 Dexterity saving throw, taking 36 (8d8) acid damage on a failed save or half as much on a successful one. The area becomes difficult terrain for 1 minute.

Legendary Actions

The Vorrithan can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time, and only at the end of another creature’s turn. The Vorrithan regains spent legendary actions at the start of its turn.

Warp Flesh (Costs 2 Actions). The Vorrithan targets a creature within 60 feet. The target must succeed on a DC 18 Constitution saving throw or roll on the Fleshwarping Table.

Corruptive Glare. The Vorrithan targets one creature within 30 feet. The creature must succeed on a DC 18 Wisdom saving throw or be paralyzed until the end of its next turn.

Summon Chasmfiends (Costs 3 Actions). The Vorrithan summons 2d4 Chasmfiends in unoccupied spaces within 30 feet of it. These creatures remain for 1 hour, until they are destroyed, or until the Vorrithan dismisses them as a bonus action.

4	The target’s eyes grow bulbous and grotesque. It is blinded for 1 minute.
5	The target’s flesh hardens unnaturally. It has disadvantage on Dexterity saving throws for 1 minute.
6	The target’s form collapses partially into itself. It is restrained for 1 minute. A restrained creature can repeat the saving throw at the end of its turn, ending the effect on a success.
